These forms are to be used when submitting applications for Discretionary Permits to the Department of Planning and Land Use. 

Forms are updated periodically and the current versions are available on this Numeric list or on the Alpha list by form title.

All of the forms are available for download and/or printing, after which you can fill them in by hand.
