STAKEHOLDER MEETING

June 8, 2012

Please join us on Friday, June 8, 2012 from 9:00 AM to 12:00 PM for a LTCIP Stakeholder Meeting. The meeting will be held at the County Operations Center,5500 Overland Ave., Room 120, San Diego, 92123.  There is two hour visitor parking behind the 5560 Overland Ave. building and also on the first floor of the parking garage which can be accessed from Farnham Drive. On April 16, 2012, AIS submitted an application to the Center for Medicare and Medicaid Services (CMS) in partnership with Palomar Health, Scripps Health, Sharp HealthCare and the UC San Diego Health System for the Community-Based Care Transitions Program (CCTP). If approved by CMS, the San Diego Care Transitions Partnership (SDCTP) will implement strategies that will benefit approximately 24,000 Medicare fee-for-service (FFS) patients each year within the four hospital systems and will change standards of practice to assure safe transitions and enhance the activation and self-management of patients and caregivers. An overview of the proposed program will be provided.

On April 4, 2012, San Diego County was selected as one of the four counties in California to participate in the proposed three-year Dual Eligible Demonstration. Under the proposed demonstration, dual eligible beneficiaries in San Diego County will have a choice of enrolling in Care 1st, Community Health Group, Health Net or Molina health plans. These health plans will deliver all Medicare and Medi-Cal benefits and services, including medical care, long-term care, and social supports that help dual eligible beneficiaries maintain their health and live at home as long as possible. There will be an update on the Dual Eligible Demonstration at the meeting, and representatives from the health plans will be in attendance.

While a Dual Eligible Demonstration Advisory Committee comprised of consumers and key stakeholders has been established by the health plans, input from the LTCIP stakeholders is essential to the success of the program. Your feedback on program operations, benefits, access to services, adequacy of grievance processes and other consumer protections is needed. In fact, your input on the Long Term Services and Supports (LTSS) network is requested prior to the meeting.
